If you have already filled out a new patient application form in the past and are still waiting to hear back from us, please know that we are processing your application, and you will receive an email with a link to book an appointment when there is an opening for new patients.
Due to an unexpected amount of requests, we unfortunately cannot give you a definitive timeframe of when you will receive the booking link for the first Meet & Greet with one of our physicians. We are doing our best to serve our existing patients and expanding to serve new ones.

How registration works
To meet one of our doctors accepting new patients at Grace Health Centre, fill out our new patient application form. Once we process your application, we will email you a link to schedule an appointment online.
Once you receive a link, we encourage you to periodically check the online booking portal as new appointment slots are made available every month.
Please note
By filling out this form, you are not formally accepted as a patient of Grace Health Centre. Your acceptance is determined upon mutual agreement with the doctor during your Meet & Greet appointment, and the clinic’s capacity to take on new patients at the time of form submission.

Influenza vaccines for all patients. No appointment needed for existing GHC patients.
We offer influenza vaccines for all patients starting each fall. No appointment is needed for existing patients of Grace Health Centre — just walk in during clinic hours.

Our Network
A Family Health Organization
Grace Health Centre is part of the North Downsview Family Health Organization (FHO).
This means that we will be working with a community of other family doctors in the Downsview area to continue offering the highest level of patient care.
As part of an FHO, our patients have access to same-day urgent care appointments during the day, after-hours telephone health advisory services, and weekend urgent care coverage through our affiliated clinics.

To keep an accurate health record and promote high-quality care, please call our office before visiting a walk-in clinic.
We ask that our patients avoid visiting any walk-in clinics that are not a part of the North Downsview FHO. Walk-in clinics will not provide our office with any records of the care that was provided at their location, which creates a gap in your personal health record with us.
Having a complete health record allows our doctors to ensure you receive the care you need based on accurate information. As such, please always contact our office first for any health-related issues, and in turn, we will do our best to make sure you receive access to the care you need.
By honoring this commitment, we are able to provide you with high-quality healthcare services such as same-day appointments, same-day weekend care for urgent issues, telephone visits for simple requests, and preventative care programs for cancer screening (PAP, mammography, and fecal immunochemical testing).

To better service our patients when booking appointments, your doctor has directed our medical assistants to ask you for the reason for your visit and other information. This will smoothen your clinic visit. We are committed to serving our patients in a timely manner, and will prioritize assisting those who become suddenly ill.

Preventative Care
Prevention and early detection is the best cure. Health concerns are more easily treated at an earlier stage.
Our doctors encourage you to get regularly screened for cancer once you are eligible. Below are the cancer screening tests we recommend for average-risk patients:
FIT
Ages 50–74 · Every 2 years
A safe and painless at-home cancer screening test that checks for tiny amounts of blood in your stool.
Mammography
Women 50–74 · Every 2 years
A low-dose x-ray system that checks for breast cancer abnormalities.
PAP Test
Women 25–70 · Every 3 years
A sample of cells from the cervix that screens for signs of change. Cervical cancer is almost entirely preventable.
Immunizations
Vaccinations
Grace Health Centre provides various vaccines at our clinic that help our patients build immunity towards different viruses, bacteria, and pathogens.
We offer the publicly funded vaccines (as part of Ontario’s routine immunization schedule) as well as select travel and seasonal vaccines. Your family doctor can review your immunization history and recommend what’s right for you.

Not only is your pharmacist able to send doctors Prescription Renewal Requests, some pharmacists can also help with the following.

What if a phone line is busy?

How do I request a prescription renewal?
Have your pharmacist send us a Prescription Renewal Request via your pharmacy. For renewals that cannot be handled by your pharmacist, please book a phone or in-person appointment with your doctor.
Can I see a female physician for a PAP test?
Yes. If you would like a PAP test performed by a female physician, use the booking link to specifically request a female doctor. After a hysterectomy, please discuss with your family doctor to determine if a PAP test is still required.
Do you offer same-day appointments?
Yes — for urgent issues, please call our office first. We prioritize patients who become suddenly ill, and may need to reschedule non-urgent appointments if our urgent slots are full.
